What to send

Sealed vials from the lot, in the container they shipped in. How many depends on the tests you pick — each line in the form says what it ships as.
The label claim exactly as printed — compound, mass per vial, lot number.
The name the certificate should carry as commissioning party. It is printed on the record and cannot be changed afterward.
After payment you’ll print a slip with your order number — it goes in the box, and the number goes on two sides of it.
Nothing else. No formulation, no customer list, no explanation of what the result ought to be.
